Hilton Head water temperature, month by month
Comfortable from late May to October, cold either side of it. The gap between air temperature and water temperature is what ruins spring trips.
The short answer
The Atlantic at Hilton Head runs about 84°F in July and August, around 73 to 74°F in May and October, and falls to about 56°F in February.
Most people find it comfortable to swim from late May through to the end of October. Outside that, the water is colder than the weather suggests, and that mismatch is the thing that catches visitors out.

Long-term monthly averages, cross-checked against two independent climate datasets. Individual days vary, sometimes by a lot, and a run of onshore wind can drop the temperature several degrees in a day. Check a current reading before you travel.
The thing nobody warns you about
Air temperature and water temperature are months apart
By April, Hilton Head is often in the mid to high 70s and looks like beach weather. The ocean is around 65°F, which is cold enough that most children are out of it within ten minutes.
The reason is thermal lag. Water heats and cools far more slowly than air, so the ocean spends spring running behind the weather and autumn running ahead of it. March, April and early May feel like beach months and do not swim like them.
The same lag works in your favour in October. The air has started to cool but the water is still holding summer, which is why October reads almost identically to May in the table below and yet has a fraction of the crowds.

What each part of the year actually feels like
March and April
Lovely weather, cold ocean. The beach is beautiful, wide and almost empty, and it is a genuinely good time to visit as long as swimming in the sea is not the plan. Bike the beach at low tide instead. If your group includes children who will want to be in water every day, you need a heated pool.
May
The turn. Around 73°F, which most people find fine, and the island has not filled up yet. One of the two best months of the year here.
June to September
Warm to the point of being bathwater in August. Also the busiest and most expensive stretch, and the only time you will struggle for a parking space at every public beach access on the island.
October
The other best month, and the one locals quietly recommend. Water similar to May, mild air, hurricane season winding down, and the crowds gone. If you can travel outside school holidays, this is the trip to take.
November to February
Not swimming weather. Still a good time to walk the beach, and the light is better than in summer.
The practical answer
If you are coming in spring or autumn, get a heated pool
This is the whole reason the table above matters. In March, April, and to a degree in May and October, the ocean is not going to be the water your family swims in.
A heated private pool changes a shoulder-season trip completely, and it is the difference between a house that works in October and one that does not. Most Hilton Head rentals with a pool do not heat it, and several that advertise heat charge for it separately or need a day or two of notice to bring it up to temperature.

What is the warmest month?
July and August, both averaging around 84°F.
Can you swim in April?
Briefly. Around 66°F, which adults manage and children generally do not. The air will feel like summer, which is the trap.
Is October still warm enough?
Usually, around 73°F. It is one of the best months to visit for exactly this reason.
Why is the water colder than the air in spring?
Thermal lag. The ocean takes months to catch up with air temperature, so it runs behind in spring and ahead in autumn.
Does the water temperature change during the day?
Not much, but wind does. A few days of onshore wind can pull colder water in and drop the reading several degrees.
Are there jellyfish?
Seasonally, most often in late summer. Cannonball jellyfish are common here and are harmless. Check for local warnings if you are unsure.
